Application for admission to the
college,
If you are not currently enrolled at West Kentucky
Community and Technical College you need to complete
an application
for admission.

High school transcript indicating
the applicant has or will complete a high school course
of study or a passing GED official score report,
Please contact your high school and have a transcript
forwarded to the address below.

Official transcript of all post-secondary
education,
Please contact the colleges and/or schools where you
completed any post high school training and have them
send an official transcript to the address below.

ACT Score report,
If you have not already had your scores sent to a
KCTCS college then you need to make arrangements to
either take the test or have your scores sent to West
Kentucky Community and Technical College. The ACT
website can be accessed at http://www.ACT.org/.
The last date to take the ACT test is June due to
the October 1 deadline.

Documentation of a minimum of 8 hours
of physical therapy observation and/or experience
(4 hours must be in a hospital physical therapy department)
You will need to contact physical therapy departments
or providers to arrange observation or be employed
by a physical therapy department to complete this
requirement. Each facility has its own rules about
how students are allowed to observe. Please contact
them individually to make these arrangements. Submit
documentation to the address below.

or arrange a meeting with the coordinator.

Enrolled in or have completed BIO
137, Anatomy and Physiology I, with a "C"
or higher, by the fall semester preceding admission
to the program.
